To print the claim form for a specific property, you should use the page to find the property and then generate the claim form.

Forms The forms in this section are supplementary forms, which may or may not be required.

	Affidavit of Lost Certificate If the original certificate is not available, complete the Affidavit of Lost Certificate that can be submitted in lieu of the original stock certificate.
	Affidavit of No Administration If you are claiming as an heir and there was no probated estate, complete the Affidavit of No Administration.
	Affidavit of Distributory Responsibility If more than one heir is entitled, complete the Affidavit of Distributory Responsibility (approving the usage of this Affidavit is based on the value of the property and the number of entitled heirs). By completeing the affidavit you are responsible to distribute the property to all entitled heirs of the deceased.
